DR Stefan Slater (Letters, December 5) gets close to the fundamental problem facing the NHS. The NHS was based upon supporting family doctors as the gatekeepers of the system with district nurses, hospitals, laboratories, x-ray facilities and professions allied to medicine who have all played increasing roles in our integrated and complex healthcare system fully funded by the taxpayer, free at the point of need.
